1.   Print out the Application Form.

2.   Circle Advanced Track or Certificate Track, indicate the appropriate module (1, 2, 3, or 4), and complete the rest of the application.

3.   Mail the application along with your application fee made out to Gateway Counseling Center for $35.00 to:

(Tuition is due on the first night of class)

4.   Print out the Reference Form.

5.   The reference form must be filled out and submitted directly to Gateway at the above address by one of your pastors, elders, or deacons.
